My Voice My Choice

As your Support Coordinator, we will assist you to identify the supports needed to achieve your NDIS goals. By supporting you to have control and choice in the services you access and how your needs are met.

During your plan we will assist you to monitor your NDIS supports and track your budgets to ensure you maximise the funds available in your plan. We will respond to any changes in your support needs and prepare you for your Planning Meeting.

Support Coordination Level 2

Assists Participants to understand their plan, overcome barriers accessing their chosen supports, monitor the progress to achieving your NDIS goals and signpost you to mainstream and community supports.

Assists with any ongoing adjustment or problem solving due to participant’s changing needs.

Specialist Support Coordination Level 3

Provides more intensive Support Coordination to Participants with complex needs who require multiple service providers and mainstream supports. Level 3 Support Coordination will assist Participants and their support networks to overcome complex challenges.
